Henry County Home Health, a part of West Tennessee Healthcare, is designed to help patients transition from hospital to home. Homebound patients under a doctor’s care can be referred to the program if they require skilled care at home. The staff provides physical assessment, evaluation, and instruction for both patient and family, while monitoring and reporting the patient’s condition to their physician.
